Active Acne vs. Scarring
Active acne involves excess sebum production, clogged pores, bacterial colonization (Cutibacterium acnes), and inflammatory response. Effective treatment targets one or more of these four factors — reducing oil, clearing pores, killing bacteria, or calming inflammation. Hormonal acne adds a fifth factor: androgen-driven sebum overproduction that requires specific management.
Acne scarring is a separate condition that develops when the dermis is damaged during the healing of inflamed breakouts. The body's repair process produces either too little collagen (atrophic/pitted scars — ice pick, boxcar, rolling) or too much (hypertrophic/keloid scars). Scar revision treatments need to break down disorganized scar tissue and stimulate new, properly organized collagen.
The relationship between active acne and scarring is critical: scarring treatments should not begin until active acne is controlled. Treating scars while new breakouts are still forming is counterproductive — you're repairing damage while simultaneously creating new damage.
