Active Acne vs. Scarring

Active acne involves excess sebum production, clogged pores, bacterial colonization (Cutibacterium acnes), and inflammatory response. Effective treatment targets one or more of these four factors — reducing oil, clearing pores, killing bacteria, or calming inflammation. Hormonal acne adds a fifth factor: androgen-driven sebum overproduction that requires specific management.

Acne scarring is a separate condition that develops when the dermis is damaged during the healing of inflamed breakouts. The body's repair process produces either too little collagen (atrophic/pitted scars — ice pick, boxcar, rolling) or too much (hypertrophic/keloid scars). Scar revision treatments need to break down disorganized scar tissue and stimulate new, properly organized collagen.

The relationship between active acne and scarring is critical: scarring treatments should not begin until active acne is controlled. What type of acne scars respond best to treatment?+
Rolling scars (broad, undulating depressions) respond well to microneedling and Halo. Boxcar scars (sharp-edged rectangular depressions) improve with Halo and Pico Majesty. Ice pick scars (narrow, deep) are the most challenging and typically require multiple sessions of Halo or combination approaches. All types benefit from regenerative therapy enhancement.
How many sessions for acne scarring?+
Typically 3-6 sessions depending on scar severity and type, spaced 4-6 weeks apart. Mild scarring may see significant improvement in 2-3 sessions.
